Each time you visit a hospital or other healthcare facility, a record of your visit is made. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment, a plan for future care or treatment, and billing-related information.

CarePoint and the hospital or facility where you received health care services from our physicians, physician assistants and nurse practitioners are part of an organized health care arrangement. The hospital or facility that you visited maintains your medical records. CarePoint does not own or maintain your medical records.

Each hospital and facility has different policies and notices regarding the use and disclosure of the health information created in that facility. Please visit the specific hospital or facility’s website for details regarding the use and disclosure of your medical records and other health information maintained by the facility.
